What’s included

  • An eight-page advance healthcare directive in editable Word and printable PDF formats
  • Sections for treatment refusals, requests, values and preferences
  • An optional designated healthcare representative section
  • Signature and witness sections, with completion and sharing notes

Start with a conversation

Think about what matters to you before filling in the form. Discuss the treatments and circumstances you have in mind with a healthcare professional. Clear wording helps the people caring for you understand the decisions you have made.

The pack contains an editable Word document and a matching PDF print copy. Work through your details, treatment decisions and any representative appointment, then follow the signing and sharing instructions. Keep the signed document where it can be found and tell the people who need to know about it.

A living will and a last will do different jobs

A living will concerns healthcare decisions during your lifetime. A last will and testament deals with arrangements after death. An enduring power of attorney is another distinct part of planning ahead. Buying one document does not put all three arrangements in place.

Is it right for me?

Adults planning ahead for treatment decisions, with support from a healthcare professional where needed. No treatment choice has been selected for you; take time to understand and record your own wishes.

BEFORE YOU CHOOSE

Your questions, answered

Is “living will” the same as an advance healthcare directive?

“Living will” is a familiar term. Advance healthcare directive is the Irish legal arrangement discussed here. Our plain-English guide explains its purpose and the distinction from a last will.

Are treatment requests and refusals treated alike?

No. Their legal effects differ. A refusal must meet the applicable requirements and fit the relevant circumstances; a request is not a binding demand for treatment. Discuss the choices and wording with your healthcare team. See the official DSS guidance.

Does the download include medical or legal advice?

No individual advice or review is included. The pack provides a structure and completion notes, but your choices need to reflect your own circumstances. Seek professional help where needed before signing.

Can I change the blank document on my computer?

Yes. Use the editable Word version in a compatible editor. The PDF is supplied as a print copy. Completing or editing a file does not itself complete the signing and witnessing process.
