A CLOSER LOOK

What’s included

  • A general power of attorney form
  • A limited power of attorney form for specified authority
  • A notice of revocation for an ordinary power
  • Editable Word versions, matching PDFs and completion guidance

Be clear about what you are authorising

Choosing the person is only the beginning. Consider the task, the limits of the authority and the information the recipient will need. Read the whole form before signing, and check any requirements with the organisation that will rely on it. Do not assume that every bank, transaction or circumstance can use the same wording.

The documents arrive in one ZIP download. Complete the Word version in a compatible editor, or use the PDF print copy. Keep a record of the signed document and who has been given a copy.

Planning for a time when you cannot decide?

That calls for a different arrangement. This pack does not create an enduring power of attorney and does not replace the Decision Support Service application. Read our guide to Irish EPAs and the DSS application steps. The term “lasting power of attorney” is used in England and Wales; it is not the name of these Irish ordinary forms.

Is it right for me?

You need to arrange authority for someone to act for you while you can make your own decisions. A limited power may be appropriate for a defined matter. Decide what authority is needed and take advice before granting broad powers.

BEFORE YOU CHOOSE

Your questions, answered

What is in the pack?

Three documents: a general ordinary power of attorney, a limited power of attorney and a notice revoking an ordinary power. Each is supplied in editable Word and printable PDF format, with completion guidance.

Does this create an enduring power of attorney?

No. An Irish EPA is a separate arrangement made through the applicable Decision Support Service process. Use our comparison of powers of attorney to understand the distinction before choosing a document.

Will someone check the authority I have written?

The download does not include individual legal advice or review. If the proposed authority is broad, the transaction is unusual or the receiving organisation has special requirements, speak to a solicitor before signing.

Planning for a future loss of capacity?

An enduring power of attorney follows a different process. Our dedicated guide explains the DSS application and where to get help.
