A CLEAR WAY TO RECORD YOUR DECISION

What this form helps you do

A clear written notice helps identify the directive you are withdrawing. Use it to support communication with your healthcare team and anyone who holds a copy of your previous wishes.

  • Identify the directive you wish to withdraw
  • Put your decision in writing
  • Keep a dated record of the change
  • Record who should receive the notice

Keep your care team informed

Discuss the change with your healthcare professionals and tell your designated healthcare representative, if you have one. Make sure those who hold copies know that the earlier directive has been revoked.

A paid template is not required to revoke a directive. The Decision Support Service provides free guidance. You must have capacity to revoke it, and revocation must be in writing.

Frequently asked questions

Is a paid form compulsory?

No. This notice is an optional way to organise the information. Free official guidance is available from the Decision Support Service.

Does revoking a Living Will change my Last Will and Testament?

No. A Living Will concerns healthcare decisions. Your Last Will and Testament deals with your estate after death; changing one does not change the other.
